Parent and Student Signature Forms
All student and parents are required to fill out their respective form. All Reyburn and Clovis East Band, Percussion, and Guard students will fill out the Band/Guard Form. All Reyburn and Clovis East Orchestra Students will fill out the Orchestra Form. These forms are for our record that you understand and agree to our guidelines, policies, and expectations for participation in the Reagan Educational Center Instrumental Music Programs.
