Medication For Student Trips/Off-Campus Activities Form

Please download and print the following form to have completed by your parent/guardian and physician for each student. This document is necessary for any and every medication, including all over the counter medications, vitamins and prescriptions for any overnight travel with RECIM. We encourage your doctors to list the end date as the last day of that school year so that this may be used throughout the entire school year.

Once the documents is completed and signed by your physician, please return to either Mr. Downs (Band/Guard) or Ms. Gilroy Golden (Orchestras) by the provided deadline for your trip. You may also email the form to your respective teacher or fax to (559) 327-4190 with attention to Scott Downs/REC Bands & Guards or Ali Gilroy Golden/Orchestras.
